COVID-19

The safety and well-being of our patients and team members always come first at FORM Physical Therapy.

We are continuing to see clients in the clinic. We are following CDC and Health Department guidelines and recommendations in regards to the COVID-19.  FORM Physical Therapy is monitoring all updates from state government and the CDC in order to provide the best care and communication during this time. FORM Physical Therapy is using an overabundance of caution to protect the health and safety of our patients, team members, and community.

WHAT YOU SHOULD KNOW:

  • Successful implementation and completion of your plan of care are important. FORM Physical Therapy is currently open and continues to serve our patients and community. 

  • For your safety, the safety of other patients, and the safety of our team members, we ask that you reschedule your appointment if you: 

    • Have any of the COVID-19 symptoms listed on the CDC website.

    • Have been in close contact with someone who has Coronavirus (COVID-19)

  • Our team members are taking extra precautions in addition to normal protocol to maintain a safe and healthy treatment environment. This includes, but is not limited to employee education on appropriate sanitation of the treatment environment, proper hand hygiene, use of face masks, and a requirement to stay home when showing symptoms of illness.

  • We are also making the following accommodations to limit potential exposure:

    • Asking that patients wash or sanitize their hands upon entering the clinic.

    • Use of face masks when appropriate.

    • Maintaining a staff that has been vaccinated.

STAYING WELL

The most effective way to stay healthy and minimize the spread of infection is to follow best health practices as recommended by the CDC :

  • Wash your hands often with soap and warm water for at least 20 seconds, especially after going to the bathroom; before eating; and after blowing your nose, coughing, or sneezing.

    • If soap and water are not readily available, use an alcohol-based hand sanitizer with at least 60% alcohol. 

  • Cover your cough or sneeze with a tissue, then throw the tissue in the trash.

  • Avoid touching your eyes, nose, and mouth with unwashed hands

  • Stay home if you are sick.

  • Clean and disinfect frequently touched objects and surfaces using a regular household cleaning spray or wipe.

  • Follow CDC’s recommendations for using face masks.

  • Get vaccinated
